Rather than having to complete a lengthy intake application, we should focus on what do they want, what do they need, and what are they hoping for.
  • The Thomson’s need a single connector/navigator and this role needs to be funded (e.g., similar to case management funding by Medicare/Medicaid)
  • `Provider compliance with regulatory and funding requirements may be an obstacle that needs review
  • We need to break down silos!
