The National Academy of Medicine (NAM) piloted the model in collaboration with five diverse communities across the United States from 2018 to 2019. Each community's plan as well as lessons learned are shared.
As a group we can definitely glean from these approaches and work towards greater structures of health equity in across diverse communities.
