The purpose of the toolkit is to provide action-based strategies and concrete steps for individuals, communities, and health care facilities to promote and sustain a Culture of Health. 

The Center to Champion Nursing in America Health Equity Toolkit is designed to guide and support nurses in their efforts to address social determinants of health (SDOH) and to advance health equity in their communities.
