Patricia Herrera M.S., Program Director for Care Coordination at 211 LA County. She has a Masters degree in Counseling from California State University Northridge. 211 LA County serves over 500,000 individuals Ms. Herrera was instrumental in designing the 211 LA Early Childhood and Care Coordination Program and in conducting the fund raising necessary to launch the project in September 2009. Program was funded by the W.M. Keck, Weingart, Mark Taper Foundations and a Robert Wood Johnson Foundation national grant. She was responsible for the identification and development of the unique MOU partnerships with community-based service partners that are key to the ongoing success of the program in both securing care for program participants and in generating valuable outcomes data. Prior to her employment with 211 LA County, Ms. Herrera worked nearly 20 years for Frank D. Lanterman Regional Center (FDLRC), one of the 21 centers in California which administers federally-funded Early Start Programs as well as state-funded programs for people with developmental disabilities. Ms. Herrera’s work experience include service disparities in underserved populations, service utilization and system approaches to improvement of health care.
