Sarah C. DeSilvey, FNP, balances practice as a rural family nurse practitioner with advocacy for addressing the social determinants of health in clinical and community practice. Her area of focus is developing the language and terminology to capture the process of caring for patients’ social needs. Over the last decade she has collaborated with colleagues at Children’s HealthWatch, the Food Research and Action Center and SIREN on medical and social care integration and interoperability projects such as "An Overview of Food Insecurity Coding in Health Care Settings: Existing and Emerging Opportunities", and the "Compendium of Medical Terminology Codes for Social Risk Factors". Her work has now evolved into The Gravity Project – a national consensus initiative to develop terminology to address social needs
